If you are visiting the clinic for the first time, please bring with you the following:

We ask all new patients to bring one form of I.D. with them. See below which I.D. is accepted.
If you are registering an under 18 patient, we will need to see one form of I.D. per person for both Adult & Child.
For all patients, any form of written past medical/vaccination history would be helpful, but is optional.

Accepted I.D. for Adult Patients:
